By signing this form, I acknowledge and agree to the following:

1. I understand that I am voluntarily participating in the NeuroBody: Her Reset workshop organised by Bhumi Lifestyle, which may involve gentle movements, light touch, positioning techniques and physical activity.

2. I understand and acknowledge that photography and/or videography may take place during the workshop for documentation, educational, promotional, and marketing purposes. By participating in the workshop, I consent to the use of photos and/or videos in which I may appear for the company’s website, social media, promotional materials, and related publications. If I do not wish to be photographed or recorded, I understand that it is my responsibility to inform the organiser prior to the start of the workshop.

3. I acknowledge that participation in any physical or movement based activity carries inherent risks. These may include but are not limited to, muscle soreness, discomfort, aggravation of pre-existing conditions, injury, or other physical complications. I accept full responsibility for any risks associated with my participation, except where caused by negligence on the part of Bhumi Lifestyle.

4. I declare that I am medically and physically fit to participate in this workshop. I confirm that I have no medical condition, injury, illness, or physical limitation that would prevent my safe participation or I have obtained clearance from a qualified medical practitioner prior to attending.

5. I understand that Neuro Reset is not a medical treatment, diagnosis, or substitute for medical care. It is intended for general wellbeing, movement improvement and nervous system regulation only.

6. I agree to immediately inform the instructor or Bhumi Lifestyle staff if I experience pain, dizziness, discomfort or any unusual symptoms during the session.

7. I acknowledge that instructors and studio staff are not medically trained professionals and are not qualified to assess my medical condition or fitness for participation. I accept full responsibility for monitoring my own physical limits.

8. I consent to instructors using tactile cueing or physical contact for guidance and correction purposes during the workshop. I understand that I may withdraw this consent at any time by informing the instructor.

9. If I have a pre-existing injury, medical condition or physical limitation, I confirm that I have disclosed this information to the instructor prior to participation.

10. I release and discharge Bhumi Lifestyle, its owners, management, staff, instructors and representatives from any and all claims, demands or causes of action arising from my participation, including personal injury or property damage, except where prohibited by law.

11. I confirm that I have read and fully understood this consent and waiver form, and I voluntarily agree to its terms.
